Do not disturb turns on automatically
When I hard restart my phone. Or if it turns off. Do not disturb is turned on.

For Do Not Disturb, let's make sure that there's not a schedule set.
Go to Settings > Do Not Disturb > Scheduled. Is this off? or if it's on, is it set up to your preference?
For Do Not Disturb to always be set to off, you need Manual and Scheduled to both be off.
Next, scroll down to the bottom of the Settings > Do Not Disturb page.
For Do Not Disturb while driving, how is the Activate section set?
Try setting this to Manually if it is set to anything else.
