I started using notes since iOS11 mostly for collecting links and images from the web but almost always when i create a new note a duplicate is created both on my Mac and iPhone.
Not only that, when I share multiple items to the same note the content gets duplicated inside as well.

If you go to source -- iCloud.com, are the notes duplicated there in the Notes app? Or are you only seeing this on an iOS device?
I have only iCloud account. Local notes are disabled. The duplicate appears is iCloud folder and Notes folder on both iOS and macOS. As you can see on screenshots above the content gets also duplicated inside notes itself.
