The free 5GB is per iCloud account, not device. Obviously you want all three devices signed into the same account, so if 5GB isn't enough you will need to purchase more. 50GB is $0.99/£1.79 per month (it replaces, not adds to, the free 5GB).
iCloud storage is on the server and is accessible to all devices signed in to the account. It is not storage on the device itself - this is not upgradeable. And I don't know where you got $12 from - the 50GB storage level is $0.99 per month (for the account, not per device). Higher levels are available: 200GB is $2.99 per month, 2TB is $9.99 per month.

I'm not sure about that, but I imagine that the Store will use your gift cards first and then charge any excess to the credit card.

If you are referring to space, a TB or terabyte is 1000GB. so 2TB is 2000GB, versus 200GB, a difference of 1800GB.
