I received a new iPhone last year as the battery was at fault. I think the same had happened to the phone I was given as a replacement. I just want to know I this one can now be replaced or if there is anything I can do to help with the battery??

The warranty on a replacement device is the greater of the remaining time on the original device warranty or 90 days.
Take the phone to the Genius Bar to discuss options.
If you get your iPhone replaced last year because of a fault and it was still under warranty. Does the warranty start again for the new phone?
