If you go to support.apple.com you can choose from several common issues that people have with iTunes accounts. You will have the ability to talk on the phone, chat over the computer, or email Apple Support to help you with any iTunes account issues.
iTunes will require a form of payment if you want to purchase anything from the store. You can create an account and then remove your payment method from iTunes after your Apple ID has been created. You can do this from the current version of iTunes by clicking on Account > View My Account... > Click on Edit next to Payment Type > Click the None box at the top of the Payment Type screen.
