you have cleanmymac installed, which has been known to slow down your mac and occasionally discard parts of the running MacOS. users who use it routinely have sometimes needed to re-install MacOs because of the damage it causes.
But your fundamental problem is your Hard drive. It is developing bad Blocks and needs re-tries, sometimes hundred of re-tries, to get good data. You need to replace it as soon as you can. Your run time of 23 minutes for what should take 3 minutes cannot be explained by the adware or anything else you are running.
To remove the possible adware, run the report again and click on the red [remove] links, then do a restart to finish the removal process.
