This last update is driving me freaking crazy- everyday it s a new thing that derails my work day!
I click the + sign, a window opens and I am able to select an application and click "add", but then nothing is added.

If that doesn’t fully resolve the issue for you, do you mind letting us know if you're signed in as an administrative user? Are you able to add login items in other user profiles? You can create a test user to find out: How to test an issue in another user account on your Mac
