My phone keyboard is distorted 2 out of 3 phone calls

Two out of three phone calls that I receive look like the picture that I have attached ... the keyboard has "merged" with the name of the caller, along with the fact that there are no further options available (keypad, speaker, mute, etc). Is anyone else having this issue? Does anyone know how to fix it?
