Hello everyone,
I have been looking around, but can't seem to find a satisfying answer. Anyone knows how I could forward/transfer/redirect an on-going phone call to another phone number on the iPhone? If the iPhone can't do it, would there be an app doing that?

No telephone can do that without a PBX switchboard or some other infrastructure hardware. And no 3rd party app can interfere with or alter the iOS telephone function and your carriers service features.
If you have a business grade service plan with your carrier, you may be able to add that option to your service. But a regular consumer handset on a regular personal cellular service plan cannot do it by default.
