create an App Specific PW, but it doesn't tell me for which App it's needed

I get a pop-up dialog saying I need to create an App Specific PW,

but it doesn't tell me for which App it's needed, so I don't know how to label it when I create it; kinda crazy making.

iMac, OS X Yosemite (10.10.5)

Started-up the iMac, and up popped a dialog saying I needed to create an App Specific password, with no indication of which App needed the password created for it.

The box behind this is std Apple ID password request. It will not accept my Apple ID Password, keeps throwing the request back up; am certain of my PW. Could it need an App Specific PW for APPLE ID? or iCloud? Confused here.



On startup I auto-launch the following:

What else can I provide?


I guess I can disable the auto-start items one by one and restart each time to eliminate.


BTW, this does repeat with each restart.
