Received Phishing Email

I received an phishing Email, see attached when I clicked on this link it already had my Apple ID then asked for my Password. I trusted the site then it asked for apple verification - Name, addres, SS#, Birthday the bottom of the page has your actual phone number call if there was a problem with the subscription.
