I have had this problem for as long as I can remember. Is there anything I can do? Sometimes it's ok, sometimes it happens. I have no idea what causes the problem. Please help me.

A simple restart seemed to have resolved this issue for me. As i_rina said, check for any updates you may have, perform the update on High Sierra and restart. I was having this issue particularly with Activity Monitor and restart brought the icon back.
I didn't see the update when I asked the question, however I booted the mac into safe mode and then I restarted - icons appeared in normal mode. However, after one day it came back - now icons in launchpad are normal, but when I open an app and it goes t my dock - it's a generic icon. Also, when I get notifications (mail, iMessage) in my top right corner, the icon next to the text is also generic. I will try and do the software update and I'll go back here. Thanks for your replies.
