Essentially passcode's are internally encrypted so you have to restore the phone to factory settings to get back into it.

If it is actually showing disabled, then all you can do is plug it into iTunes and restore it. If you have a backup of the content on it, you can then restore that backup to it. Otherwise, at this point, yes, you are inherently going to lose all content on it.

Yes, it does have to erase everything. You can restore a backup or if you have iCloud photo library then your photos will come back once you sign into your icloud account
