iTunes Messing With Albums

I am iTunes 12.7.2.58. I've been having trouble recently with iTunes syncing with my phone, a SE. The albums look normal on my Mac, but when I sync with my phone, it with put every song in one album into individual unknown albums. For example, I have Captain America: Civil War Soundtrack by Henry Jackman. The album has 21 tracks. So when i put it on my phone, I can go to Henry Jackman, but there are now 21 unknown albums, each with only one track. Anyone have a solution to this?

I understand that when syncing music with your iPhone, some albums will be synced as multiple albums. I'm happy to provide some help for this issue.
