MBP 2016 freezes after waking up with external display

My MBP 2016 freezes after sleeping for hours when connected to an external display. I have a LG 27"4k monitor at home connected from USB-C to DisplayPort and at work, a 2k Dell monitor connected using the apple Multiport adapter with HDMI. When I open the laptop all the screens display a solid color, for example, the laptop screen is completely yellow and the other monitor completely red. Happens with any of the monitors connected. Also, a co-worker with another MBP 2016 suffers the same issue. We already tried SMC and NVRAM resets procedures and both machines are updated to the latest MacOS version. My MBP has the Radeon Pro 455 and my co-worker's only the Intel HD graphic card. Can anyone help me with this issue? So far only occurs after been slept for hours and connected to external displays.

I have the exact same issue with a 2017 MacBook Pro attached to two Dell 24" external monitors independently hooked up to USB-C on either side of the machine. I walk away from the computer and return after the monitors have gone to sleep and the system has shut down and needs to be restarted. Its been identified as a sleep wake issue but can not be resolved. Apple went to the extent of exchanging my machine for another to see if that would resolve the issue and Ive done a reinstall of software along with no back up from time machine to eliminate any possible virus issues. This appears to be an internal hardware flaw, if I disconnect the monitors before I put the system to sleep then I have no issues.

If your Mac doesn't wake when you expect

Your Mac might pause a few seconds before it wakes up. If it doesn't seem to wake at all, check for these possibilities:

  • Your screen brightness might be turned down.
  • If you're using an external display, your display might be turned off.
  • Your Mac might be in safe sleep. To wake from safe sleep, press the power button.
  • Check for a sleep indicator light, if applicable, and make sure that your Mac isn't turned off.

If the previous steps don't work

These additional steps may help identify or resolve the issue:

  • Reset the SMC.
  • Reset NVRAM.
  • Disconnect external devices other than your Apple keyboard, mouse, and display. If that resolves the issue, gradually reconnect your devices, testing each time, until you find the device that is interfering with sleep. Then check the documentation that came with the device, or contact the device manufacturer.
  • Start up in Safe Mode to see if the issue is related to non-Apple startup items, login items, or kernel extensions.
  • Try to isolate the issue by using another user account.

If you still see the issue, contact Apple Support.
