I had the issue in macOS 10.12.6 (Sierra).
Upgrading to 10.13.3 (High Sierra) solved the issue for me.
Unfortunately you have the issue in 10.13.3 so I don't have a solution for you. 😟

No luck with Ulexes138 suggestions. I even reset all the caches manually and the launch service databases to original default associations. Then I reset the PRAM and NVRAM and finally the SMC. No change.
Either it is a software issue as you had with 10.12.6 or something in broken on the hardware side. I might re-installed the 10.13.3 update from the combo installer. Other than that, looks like a phone call to Apple support to see what is in their database. Since I haven't seen much about this in bulletin boards, it looks like I need a genius with or without a screw driver, maybe.

Excuse me if I pick up this thread.
I have the same problem on my MacBook Pro (15-inch, 2016) running macOS 10.13.3 with iTunes 12.7.3. I have started in safe mode and instead of pixelated videos, I get a black window with some stuttering. After setting up a new user account (after logging back in), I get pixelation. Therefore no change. All I know is that it used to work as it still does on my MacBook Pro (Retina, 15-Late 2013) with the same software. It has passed the power on self test and none of the graphics cards show a problem.
What next? And ideas? Both graphics cards can't be gone at the same time.
