In the United States the price of a glass replacement is $149 (plus any sales tax) if it can be done. There are set prices in other countries. Other countries you would have to bring it in to find out the repair cost.

If it's more severe they'll likely replace the whole device. That's $319 in the United States.

If you can live with the scratch to the case and they say they can still do the glass repair (not always possible depending on what happened to the case) then you can do just the glass repair and maybe use a phone case to cover up the cosmetic damage.

Apple doesn't refurbish iPhones for customers. They might take in used phones (might be for battery, glass damage, returned items) with a complete swap. They'll then take the returned devices, put a new battery/glass in, and reset and condition it to factory condition.

What's the issue for which you want it refurbished? I wouldn't really recommend having anyone else try to do anything that requires opening the case. Once there's an aftermarket part in an iPhone, Apple is know to refuse to service them.

At Apple, the price is APPROXIMATELY half the cost of a new identical phone. Call them and confirm the price.

I'm glad that you posted. I had incorrectly believed that Apple would do a phone swap.
