Sorry, the iPad does not have a phone icon.

An iPad is not a cellular telephone and has no ability by itself to make cellular telephone calls. Thus it has no phone icon nor app.
No iPad has a phone icon as no iPad is a phone.
So, there is NO phone icon to restore.
Maybe you are remembering wrong?
Maybe that phone icon was for a third party phone app from the iOS Store.
No iPad ever has ever had a default phone icon on it.
iPads cannot make traditional cellular phone calls. EVER!
I have been using iPads for 8 years.
They have never had a built-in phone app as part of iOS.
I own a 64 GBs storage iPad 2.
No phone icon/app.
Make an appointment for the Genius Bar and they’ll fix it.
