ADFS signout showing as .dms file download

Noticed that after i perform a single sign out from ADFS which redirected to ?wa=wsignoutcleanup1.0, i am seeing

the page been loaded as .dms file as shown below. Do not see the same issue happening on other browser (IE, Firefox, Chrome).

Any idea?

What is APFS user agent and what would make it need attention? I'm getting a notification that the APFS user agent needs attention. I'm also getting this message -- Volume Hash Mismatch Hash mismatch detected on volume disk3s1. macOS should be reinstalled on this volume. They seem to be related. I see other discussions about the Hash Mismatch, which others not being able to reinstall the system as the notification recommends. Same here. My MacBook Air has become unstable, livestream movies freeze and Skype crashes. This instability has never happened before. Is it Monterey 12.6? Thanks in advance for community member's thoughts.

adguard/adblocker not working in mojave This started 4 days ago, one morning I started my MB Air and adguard stopped working, ads on every website. I thought maybe there was an update I didn't know about so I deleted it, reinstalled it, same thing. I downloaded adblocker to try, same thing. I ran MalwareBytes and it found 7 items that it quarantined, but the issue is still happening. Any ideas?
