iCloud Keychain Passwords will not sync/update between devices...

I've tried multiple times turning iCloud>Keychain On/Off, but the App & Website Passwwords will not sync. They do not show up on my iPad. Do I need to delete them on each/all devices including iMac so the ones stored on iCloud Keychain update all devices? I've tried keeping them on my iMac, or iPhone, and it still doesn't work.

I've also been having ongoing issues with changed passwords not updating on other devices. If I change a website password in Safari on iPad then try to log in to same website on iPhone in Safari, or app, or vice versa, I will get a incorrect password warning, even when I manually copy/paste or type in the new password. This has been happening for quite a while and I cannot figure it out.
