I've tried calling a store. Sent me straight to AppleCare where I was on hold for a half hour. The AppleCare rep was able to directly contact a store manager though, and probably has access to appointments. If the store is nearby one can go to the "point person" and look up the appointment.

The easiest way is to use the email that a person received as a confirmation, or to use the Apple Support app to shows your reservations, however the OP stated they no longer have their phone, so I'm guessing they do not have access to the email.

Call the store and they can provide you with information. This is a user to user support forum, and Apple does not participate here. No one could get you any information about your appointment.
