Restrictions turned off but still asking for passcode.

Why would I be encountering safari pages with restriction passcodes when the restriction setting is turned off? I can’t even make an appointment online at my doctor’s office because the page is restricted. I tried entering the passcode I set up for my son’s phone, but it doesn’t work. Please don’t tell me I must have turned it on, I can take a screenshot of the setting, it is plainly off. I have also added a screenshot of a restricted website.
