Not all iPhones can have the camera shutter sound muted. Phones designed for sale in certain countries cannot have that, due to local laws. One such country is Japan, so iPhones designed for sale in Japan cannot have the shutter sound muted, no matter what the user does. It is possible that your phone is one of those.
