Below the section to type your username and password, there should be blue writing that says "forgot password". Click on this. You will be prompted to enter your Apple ID. Confirm your number when asked. This is the phone number that it will send your verification code to so ensure you have access to it. It has given you the last two numbers so you must type in the entire phone number ending with those two numbers. Type in the code on your device from the code that was sent to you. Continue on and then you will be prompted to enter your new password.
