My tv app on the main screen is missing. How do I recover or where has it gone.

If you cannot locate the app, then use Spotlight search on the phone, and see if it is located. If not, you can download the app from the App Store. See this about Re-installing apps. Remove built-in Apple apps from the Home screen on your iOS 10 device or Apple Watch - Apple Support
