How I can delete or request to reset my iCloud

In past ahm**@yahoo.com was used in India, he make iCloud ID, after that I get this email and using that ID & Password to my iPhone 5, few years ago I don't why I lost my this ID and tried many time to create new with associate with this email address but I won't. In past 5 months ago I again try to create iCloud ID and now I got successfully make it but problem is I login successfully in web. But when I try to login iCloud it every time said Unlock Account but my account ID & Password obviously correct and follow instructions many time but failed.

How I can delete permanently my iCloud ID or any method to reset my iCloud ID preferences.
