Nope. No one is hacking your phone. They are absolutely not accessing your camera.
It's entirely your iCloud account has been compromised, meaning someone has access to the information in your cloud-space, including photo-stream, iCloud photo library, call history, contacts, calendar, etc.
Maybe you'd like to explain why you believe someone is accessing your camera...
If your phone is not jailbroken no one is hacking your phone.
