Could you let me know whether you are using the original power adaptor that came with your Mac? If you have another Macbook around, could you try your adaptor with that Mac. This will help determine whether the issue lies with your Power Adaptor or the Macbook itself?

Check the battery cycle count > Determine battery cycle count for Mac notebooks - Apple Support
Depending on how long you've had your Mac, it may need replacing.
Cycle Count, 157, it is a new notebook. I am really using not more than 6 moths.
