my ipod touch is disabled and was removed from our itunes account. How can I restore it and use it again?

This is an iPod 5th generation. My son forgot his password and disabled it. He tried to hook it up to iTunes and accidently removed it from the iTunes account. Now we do t know what to do next. Any help would be great.
