I wish to factory reset my iPhone 5

I wish to factory reset my iPhone 5. Pls guide how I could do that. In Settings if I go to reset all settings, it asks for a restrictions code and I don't remember that.

Restriction Code I want to factory reset an iPhone 6, and when I choose Erase All Content and Settings, it asks me for the Restriction passcode and I don't know what it is. Any help appreciated.
