"You appear to be listening on another device" when I am not!

I frequently get the message while playing music on my iPad in my office, when it is not possible that my account is "listening on another device." I listen via my iPhone in the car to work, then switch to the iPad in the office. The phone in my pocket is definitely not "listening." I have to click the warning message and then restart the music, which is annoying. Why is this happening?

We just recently updated to new phones, so I went in and removed all the old phones and back to our baseline number of devices. But the warning would not go away.
