Earphone jack and lightning connector jumps out of the port when connected.

I can no longer use earphones with my iPhone 6S because the jack does not stay inside the port. It automatically ejects the jack. Same with the Lightning connector, it does not firmly fit inside the port and has some loose connection during charging. How do I fix it?
