photo icon missing
itunes and phone icon missing, tried all previous suggestions, no good.

What versions of iTunes, OS X, iOS, and device are involved? Have you tried reinstalling iTunes? Are you running the current versions? Have you tried a known good cable? Will your device connect to a different machine?
