The Camera application cannot be removed from an iPhone. So since you don't see the icon, it is almost certainly in one of your folders. Check there.
Or you could use Spotlight to find it on the iPhone.

Thanks, so much for your reply. I have double and triple checked all folders. I tried going into a text message and try to access the camera that way and the screen goes black. When I flip the camera view the other direction, same problem. There is no camera listed under settings. Very weird!
