Just a comment. I found that I have the same issue with a icon for EasySuite on my iMac. I read the question and replies on this forum and found that a USB 3.0 Compact Memory Card Reader from INSIGNIA and its cable are the root of the problem. Removing the cable from my USB chain seems to have resolved my issue.

I’m still having this problem and this is the only solution I see on the web. How/where do I access my USB chain? I’m in Key Access in utilities but not seeing anything. Thanks!!
