Given that, until the end of the year, Apple is giving a big discount on the battery for your phone, take advantage of that while you can. I agree with ProustGulio that you should get it done sooner rather than later. Because of the discount, it can sometimes take a bit of time to get an appointment.
Check your charger it might have connectivity issue with the phone

I believe it's the same number that an official Apple diagnostic will yield, which is why I said "pretty accurate". No battery health reading is going to be perfect though, because it's still an estimate based on what's observable.

I have no problem, and your suggestion has nothing to do with the subject of this thread.
If I can add my two cents, I wouldn't wait too much to have it replaced, making an appointment at your most convenient Apple Store or Authorized Service Provider: Apple - Find Locations - Location Selector.
(don't forget to make a full backup of your iPhone before the replacement)
When batteries reach the critical point of their life, they often have unpredictable behaviors, so I wouldn't risk to see your battery dying unexpectadly (or being unable to charge).
This is only my opinion, and you're free to decide, of course.

Battery health should be pretty accurate. Apple has been keeping track of battery healthy for a long time, but didn't make it visible to the user in iOS. If it's actually 80% or under, Apple's current plan will allow the user to access the promotional price again before the program expires. It's been reported that the user gets a single opportunity to replace the battery at that price if it's ABOVE 80%.
