After running a hardware test on my iMac 21.5 late 2012 i got an error code 4MEM/61/40000000:0x7f6e9f90
What does that error code mean ?

It's a memory error. Check your RAM to be sure they are seated properly in the slots, one of them may have come loose. If that's not it, remove the modules one at a time and test to find the culprit and replace it with new RAM.
Macs are fussy about RAM. Both Crucial and OWC are reliable vendors with high quality RAM.
Thank you for your reply but for my model of iMac 21.5 RAM had to be installed by Apple from the beginning and could not be changed by the user. When I look at the status report for RAM both memory have the status OK. Could it still be problem with the RAM?
The Apple Hardware Test, or Apple Diagnostics (depending on the age of your Mac) is very good at identifying problems. If it reports problem, then you can be pretty sure there is one. Sometimes it will report no problems when there are problems.
If yours is a model without user replaceable RAM, you'll need to bring it for further testing and possible replacement.
