So I have a iPhone 6S with iOS 11.4.1 and audio is very clear in both Camera app and Snapchat.
However, when I record a story through Instagram there is this white noise and the sound is really, really muffled.
For example, as soon as I move the device a bit, there is this awful and loud noise that ruins the entire video.
What this could be?

I found the solution. Basically it's the new update that caused problems. I got to install an older version of Instagram (v44.0) and everything is working flawlessly. No more white noise and loud noises as well.

I would contact Instagram's Support team and ask them.
In my opinion, as you're specifying in your question, that issue seems to be related only to Instagram's audio recording.
I don't have an appropriate answer to offer, except saying that, even if you had your device inspected at an Apple Store, they should diagnose that your microphone is working fine (without any hardware problems).
And, since you're using the same device with different apps (camera, snapchat, voice memos, and so on) and it is working fine, the issue should be restricted only to that app.

Yes, the problem takes place only with Instagram.
I just checked Whatsapp recordings, Voice memos recordings, Snapchat, Camera and so on and the audio is very clear without any noise
I'll definitely try to contact Instagram support.

You can hear this white noise in background and as soon as I zoom in with my finger, you can clearly hear it.

Is it possible that an older version might work? I've read somewhere that starting from v45.0 the app gave this problem.
I don't know how to install an older version (in this case v44.0 or lower) though š®

Hummm, non credo tu possa scegliere di installare una versione "vecchia" di Instagram, a meno che Instagram stessa nonte la metta a disposizione.
