how to unlock activation code for my iphone 4s

I can't sign in to my iphone 4s as I forgot my apple id & password hence my phone has been locked and asking for Activation code but for that I have to type my e-mail and password for icloud, I tried all the passwords but its not working.

iPhone 4S, iOS 8.1.1
