Pop-Up Window asking for me to sign in

This window suddenly popped up on my screen - anyone know what is going on here? Doesn't feel right.

What app were you using when messsage appeared?

What is the app in the upper left of the screen, to the right of the apple icon?

You are correct to be suspicious. The wording to me seems off. I think the message should include what app is requesting the change. Oh..., it's a bogus message. The word Apple would not be in the message.

What were you doing before the message appeared? What is the app in the upper left of the screen, to the right of the apple icon?

How might they have gotten your name?

So far it has popped up a couple of time in the middle of my screen. If I hit cancel, at first it does nothing, then if I hit again it goes away.
