The MacBook Pro from Mid 2009 can be updated to OS X El Capitan 10.11.6.

iTunes can be updated to the latest and current version 12.8, which requires Intel CPU, and OS X version 10.10.5 or later.

iTunes 12.8 may request another internal update if you connect (tether) an iOS 12 device. This update is required to connect to the iOS device.
