Was this hardware previously working?

Has the storage-related hardware possibly failed?

Any recent hardware changes?

Does Disk Utility show any connected storage, from the Utilities section of the first screen of the installation process?

Does wiping the internal storage (using Disk Utility) and returning to this display show a connected device?

Does connecting an external, scratch USB, FireWire or Thunderbolt storage device show a target device available when returning to this screen?

  1. What prompted the reinstallation of macOS?
  2. Is “Macintosh” the name of the internal storage, as seen in Disk Utility? If so, what storage volume format was used for that storage when it was initialized? (Disk Utility usually shows what format was used.)
  3. Does connecting external storage and restarting the installation then show up?
  4. Which Mac? How old? SSD, HDD or Fusion storage?
  5. Which version of macOS is being installed?
  6. Have there been any recent hardware changes? Has the system been damaged?
