It's still an attachment, as I stated:
This first photo shows you the three places where it indicates that there is an attachment for this email. The bar in the middle only becomes visible when you scroll over that area with your cursor:
The second photo below shows you the dropdown when click on the arrow next to the attachment indicator in the middle section:
And just so you know, Apple is not here. This is an all-volunteer user to user support forum, so no one from Apple is here.
Also tapping Control and clicking on the photo provides the same options:
On an iPhone, you tap and hold the image until it displays with the image name at the top, then tap on the "share" button and save it to any number of places - iCloud Photo Sharing, iCloud Drive (Files), your Camera Roll (Save Image), etc.

No, they are not attachments. They are inserted into the body of the email, instead of being sent as an attachment.
This is honestly quite ridiculous. I've seen this question asked so many times and it's always the same responses, "it is an attachment" or "its the email client you're using".

It's the Apple email client. No matter what type email address you send from (Outlook, Gmail, iCloud, etc) and no matter what the client receiving the email is (Outlook, Gmail, iCloud), ALL photos are sent with the photos displayed in the body of the email, instead of the body being blank with the pictures ATTACHED to the email. Which is SUPER annoying, as it makes saving photos from emails a nightmare.
This is so frustrating. Pictures used to be sent as attachments, now they all display in the email body. Why is there no one who can explain this? Why is every comment thread on these questions full of people denying this is happening?
Can someone from Apple please explain why the Apple mail client does this?
