Data Entry Form for iPad Pro

Hi, I'm just wondering if anyone can pass along advice on the best method/software to create a data entry form for an ipad. I've been looking around and see Numbers has a basic solution to convert tables to forms, there are also a gazillion apps that purport to do the same.


The situation is as follows: (they are asking for a laptop with excel to do this but I'm trying to get a better solution together): a geologist inspects rocks, and with wet/dirty hands enters 15-20 variables of different types (mostly numbers, maybe a date, and a text note), each record entry could take 1 minute... then they move on to the next set of rocks and create a new record. Internet is not available in this location, but it is in a building so the environment is not entirely uncontrolled. Hands will be wet, but can be somewhat dried before each record is entered.


I had an app way back called tapforms which i see is still a thing... haven't used it for anything in ages though. Any advice is appreciated!
