If you are using iOS 11, Settings->General->Restrictions, if using iOS 12, Settings->Screen Time->Content & Privacy Restrictions and see if you have Apps restricted by age or by content type there.
Restrictions do not set themselves, you or someone else needed to have set them. You will need to remember the passcode you used when setting up restrictions. Otherwise, you will need to restore the iPhone to remove the restrictions.

This was helpful, but I have no restrictions set. I went to Settings>Screen Time>Content & Privacy Restrictions and restrictions were off. I am still unable to access my Amazon app.

You will need to restore your device as New to remove a Restrictions passcode. Go through the normal process to restore your device, but when you see the options to restore as New or from a backup, be sure to choose New. You can restore from a backup only if you have one from BEFORE you set the restrictions passcode.

Follow these steps to place your iOS device into recovery mode. If your iOS device is already in recovery mode, you can proceed immediately to step 6. 1. Disconnect the USB cable from the iPhone, iPad, or iPod touch and leave one end of the cable connected to your computer's USB port. 2.
