Does it show on the purchase history of any iTunes account you've got linked to the card : See your purchase history in the iTunes Store - Apple Support ?

If not, and if it's not a About payment card authorisation holds in the iTunes Store - Apple Support, talk to the card issuer.

If it does show, and if you claim that neither you nor anyone you've given access to the account did it (I assume you've already changed the account's password if you are saying that it's been hacked), contact iTunes Support and see what they say (you know that you are only talking to fellow users on here) : http://reportaproblem.apple.com or http://www.apple.com/support/itunes/ww
