Phishing Email from "Apple Support"

So not so long before I post this thread, I received an email stating that my account is temporarily disabled for security reason as shown below

It is also stated that I should open the attachment inside the email and follow the instruction inside.

So 'cause of my curiosity, I download the attachment and open it (I download it on Kali VM machine, in case something bad happen). The attachment looks like this:

In short, the PDF asked me to click on the button "Click Here". Before I click it, I hover the button to show the link that would be opened if I click it. Does the link seem legit? It's a big **** NO. But well, I open it right away, and it looks like this:

I'm a bit sad 'cause I can't explore more from this email (since I rarely got this kind), but here it is

After all this happened, I came up a few question:

1. What does the sender want? (since I can't continue further)

2. Does my email (or our email) got leaked from Apple Database?

3. If so, what happened?

An answer might be useful, since some common people with no idea what is happening can be a victim from this kind of attack.

It is phishing. What they want is your personal information. It wasn't leaked from Apple database, it was gotten elsewhere. In a message Apple will always address you by your real name, never by your email address or Dear Customer. Also the sent-from address should be @apple.com or @itunes.com Additionally Apple does not send attachments. And note the poor English.
