I purchased an app that has a monthly subscription that I need to renew. Do I need to keep a balance so they can just draw from my iTunes account?

You can either keep a balance on your account or have a valid payment method (e.g. a credit card) on the account - the subscription payments will be automatically taken (from the balance if you have on, otherwise the card)
