I see that you were logged out of your Apple Music subscription. Now, the only option you have is to join Apple Music but you'd like to rejoin your Apple Music Family subscription. I'm happy to help.
Turn on iCloud Music Library with Apple Music -- If you're signed into your iPhone with the same Apple ID you were using before, tap Settings > Music and make sure "Show Apple Music" and "iCloud Music Library" are both turned on.
