iOS 12 requires iTunes 12.8 or better running on OS X 10.11.4 El Capitan or better.

If required see How to upgrade to OS X El Capitan - Apple Support to check if upgrading is feasible.

If you can't/won't upgrade then a workaround for music is to use iTunes Match or Apple Music, and use iCloud for backup and data transfer.

If you're already using iTunes 12.8 download the current 12.8 installer for iTunes from https://www.apple.com/itunes/download/.

Unlock your iOS device and connect it to USB.

Install iTunes from the .dmg that you've downloaded.

During the installation it should announce that a further software update is required.

Install that and iTunes should see your device.
